RULE 756(j) PRO BONO AUTHORIZATION FORMS FOR INACTIVE, RETIRED
AND HOUSE COUNSEL LAWYERS AND SPONSORING ENTITIES

The following forms are provided for use by lawyers and pro bono legal service entities in connection with Illinois Supreme Court Rule 756(j). 

Form:  Attorney 1
For use by inactive, retired and house counsel lawyers seeking authorization to provide pro bono legal services under the auspices of a qualified sponsoring entity. 

Form:  Attorney 2
For use by inactive, retired and house counsel lawyers seeking annual renewed authorization to provide pro bono legal services under the auspices of a qualified sponsoring entity.  The second section of the form is to be completed by a representative of the entity sponsoring the pro bono legal service program in which the lawyer participates. 

Form:  Sponsor 1
For use by not-for-profit legal service organizations, governmental entities, law school clinical programs, and bar associations, seeking to serve as sponsoring entities for inactive, retired and house counsel lawyers to perform pro bono legal services.

Form:  Sponsor 2
For use by not-for-profit legal service organizations, governmental entities, law school clinical programs, and bar associations, submitting annual verification of the continuation of pro bono legal service programs in which inactive, retired and house counsel lawyers participate.

Notifications from lawyers and sponsoring entities of the end of a lawyer’s participation in a pro bono program may be made by letter and should also be mailed or faxed to the above address/fax number.
